Editorial and Content Review Policy

HCA publishes educational information to help you understand insurance terminology, prepare questions and locate primary sources. Our articles do not replace current plan or policy documents, government eligibility decisions, or legal, tax, medical or individualized investment advice.

How we develop educational content

  • Begin with a defined reader question and answer it in plain language.
  • Use primary government, regulator and carrier sources for rules, dates, benefits and costs.
  • Distinguish general education from a recommendation based on an individual's circumstances.
  • State HCA's represented-market limitations where they affect the reader's decision.
  • Do not publish invented reviews, credentials, office locations, service areas, plan availability or first-hand experience.
  • Use examples only when they are clearly labeled and do not imply a guaranteed outcome.

Authorship and professional review

Articles may be prepared by the HCA Insurance Editorial Team using primary sources and existing HCA subject material. A named professional reviewer will be displayed only after that person has substantively reviewed the page and approved attribution. Laura's profile and any reviewer credentials will use only facts approved for publication.

Sources and links

We prefer Medicare, Social Security, New York State, federal regulator and current insurer documents for factual rules. Sources are linked near the relevant material or collected in a clearly labeled source section. A link does not mean that the outside organization endorses HCA.

Dates and review cadence

A published or reviewed date should represent substantive work, not a cosmetic change. Medicare, Affordable Care Act (ACA), Social Security and other year-sensitive pages should be reviewed when official annual information is released and before seasonal promotion. Evergreen pages also require periodic review for broken links, changed terminology and scope.

Corrections and material updates

Send correction requests to info@hca-insurance.com with the page URL and supporting source. HCA will evaluate material issues, correct verified errors and update the review date when the change is substantive. Pages that cannot be maintained may be removed from the XML sitemap, marked not to be indexed or redirected to a stronger resource.

AI-assisted editorial work
